LAUNCHING NEW FREE PROGRAM “HOME CARE FOR DEMENTIA 56 PLUS”
SEPTEMBER 2019 – OCTOBER 2020

Aware of the negative effects of dementia and related disorders on individuals’ families and in the community, the Greek Society for Alzheimer’s Disease and Congenital Halkida Diseases has created the free program “Home Care for Dementia 56 PLUS” for integrated education carers of people with dementia who for various reasons are unable to move.

The program has been running since September 2019 with an exclusive donation from TIMA Charitable Foundation. Family caregiver training is conducted through home visits and lasts 6 months with the aim of better managing the needs of the person with dementia and reducing the burden on the caregiver and is carried out by an interdisciplinary team comprising: Case Manager-Psychotherapist , Psychologist, 2 Nurses and Physiotherapists.

The program will provide each family with its needs:
  • Information on Alzheimer’s disease and other dementia.
  • Disease education and counseling in the treatment of patient behavioral disorders.
  • Training for personalized patient care.
  • Training for future planning / planning.
  • Specialized information on legal, insurance and other financial matters.
  • Psychological support for relatives of patients with dementia to cope with the effects of such care.

Interventions in the ergonomic layout of home furniture / rooms to make it easier for the patient and their carers.
At the same time, within the framework of the Program, the “Chalkida Counseling Station for Dementia” will operate three times a week with the aim of timely counseling on memory problems, and the implementation of mental and physical empowerment programs. The Counseling Station will operate in the areas of Halkida and Mantoudi.

Specifically will be provided:

  • Tests for the early detection of memory and mood disorders and counseling / referral for a diagnosis by a Neurologist or Psychiatrist.
  • Monitoring people with dementia and carers through psychometric tests.
  • Information on Alzheimer’s disease and other dementia.
  • Disease education and counseling in the treatment of patient behavioral disorders.
  • Training for personalized patient care.
  • Training for future planning / planning.
  • Specialized information on legal, insurance and other financial matters.
  • Psychological support of relatives of patients with dementia to manage the burden of burden.
  • Family mediation.
  • Interface with the community.
